Job summary

Kadima Health and Nursing at Lititz is seeking a Physical Therapist to join our care team in Lititz, PA. The role focuses on evaluating and treating residents to improve function and reduce pain, following a formal plan of care.

The successful candidate will possess a PA Physical Therapist license in good standing and be able to document patient progress and coordinate with families and caregivers. Prior SNF/long-term care experience is preferred.

Qualifications

• SNF/Long term care experience preferred.
• Current knowledge of treatment practices.
• Knowledge of Net Health Electronic Documentation System preferred.
• Ability to manage patients with different personalities.

Responsibilities

• Evaluate, plan and administer treatment to residents to relieve pain and restore function.
• Ensure all treatment delivered per plan of care.
• Provide clinical support and instructions to patients, families and caregivers.
• Measure and record patient’s motor function, strength and muscle performance.
• Monitor patient responses and modify treatments as needed.
• Complete documentation per company policy and state/federal regulations.

The Physical Therapist is expected to follow facility core values in administering department services and helping to fulfill all therapy‑related resident care needs.

Duties/Responsibilities:

• Evaluate, plan, and administer treatment to residents and patients with temporary or long‑term disabilities to relieve pain, restore or improve function, and promote healing.
• Assured all treatment is delivered in accordance with an established plan of care.
• Provide clinical support and instruct patients, families, and caregivers.
• Measures & records patient’s motor function, strength, and muscle performance.
• Monitors patient responses to treatments and makes modifications during treatment sessions as necessary.
• Completes documentation in accordance with company policy and state/federal regulations.

Skills:

Required licensure:

• Current License as a Physical Therapist in the State of PA. Must be in good standing.
