OSR Physical Therapy in Norwood Young America, MN is seeking a strong Physical Therapist to join our friendly, family-oriented clinic. You will assess patients, review referrals, provide direct care, document progress, and coordinate with physicians and insurers to ensure optimal outcomes.

Qualifications

• Graduated from a CAPTE accredited Physical Therapy Program.
• 1+ years of experience; New/Recent grads welcomed & encouraged to apply.
• Current state of Minnesota license and CPR certification.
• Previous outpatient orthopedic clinic experience preferred.
• Outgoing and energetic personality.

Responsibilities

• Assess patients to develop and initiate treatment plans based on evaluations.
• Review physician referrals and records to determine required PT treatment.
• Provide direct patient care per established protocols and guidelines.
• Document findings, progress, and instructions to patients and caregivers.
• Oversee care performed by assistants and rehabilitation technicians.
• Complete write-up of care, set goals, and develop comprehensive programs.
• Implement PT treatment programs and provide essential therapies.
• Communicate with patients, caregivers, physicians, and insurers.
• Ensure timely documentation including daily notes, progress reports, and discharge summaries.

Job Description

OSR Physical Therapy has an immediate opportunity for a strong and enthusiastic Physical Therapist to join our fun, friendly, and family-oriented clinic in Norwood Young America. • Assess patients to develop and initiate treatment plans based on the evaluation results
• Review the physician's referral (prescription) and the patient's condition and medical records to determine the physical therapy treatment required
• Provide direct patient care by established protocols and patient care guidelines
• Document findings, progress, and instructions to patients and caregivers
• Responsible for patient care performed by assistants and rehabilitation technicians
• Complete a written plan of care, develop goals based on evaluation findings, and develop a comprehensive program to attain goals
• Implement physical therapy treatment program and provide essential physical therapy treatments
• Communicate effectively with patients/caregivers and professional colleagues, including physicians and insurance companies
• Ensure timely documentation, including daily notes, weekly workers' compensation reports, progress reports, initial evaluations, discharge summaries, and incident reports

Additional Information

The anticipated base pay range for this position is $32.00-$38.00. Payis based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography. Additionally, this position is eligible for discretionary incentive compensation. The Company's incentive compensation plan is subject to change. Medical, dental, vision, 401(k), paid time off, and other benefits are also available, subject to the terms of the Company's plan.
