Job summary

Enhance Rehabilitation is seeking qualified Physical Therapists to join our team in SNF settings. You will evaluate and treat patients following individualized plans of care, collaborating with facility staff to provide high-quality care.

You will supervise Physical Therapist Assistants, re-evaluate progress, and maintain thorough documentation of evaluations, re-evaluations, discharges, and daily notes.

In this role, you will evaluate and treat patients following an individualized plan of care.

• Providing excellent care for our patients

• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
