Job summary

Enhance Rehabilitation in Crystal Lake, IL is seeking qualified Physical Therapists to join our SNF care team. You will evaluate and treat patients, develop individualized plans of care, and set measurable goals to restore function.

You will collaborate with facility staff, supervise Physical Therapist Assistants, document evaluations, progress notes, and discharges, and educate patients and families on injury, progress and outcomes.

• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including evaluations, re-evaluations discharges and daily progress notes
