Job summary

Enhance Rehabilitation in Milwaukee is seeking qualified Physical Therapists to join a growing team providing high-quality care for patients in SNF settings. As an Enhance Rehabilitation Physical Therapist, you will evaluate and treat patients following an individualized plan of care.

You will provide excellent patient and family service, collaborate with facility staff, develop plans of care with measurable goals, educate patients on progress and outcomes, supervise Physical Therapist

Qualifications

• Qualified Physical Therapist able to evaluate and treat patients.
• Experience in SNF settings helpful.
• Ability to develop plans of care and set measurable goals.

• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
