Job summary

Enhance Rehabilitation is actively seeking qualified Physical Therapists to join our dynamic team in a SNF setting. You will evaluate and treat patients following an individualized plan of care, aiming to improve functional outcomes.

You will collaborate with facility staff, provide excellent patient and family service, supervise assistants, and maintain thorough documentation of evaluations, progress notes, and discharges. Previous SNF experience is a plus.

Responsibilities

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent patient care
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
