Job summary

Enhance Rehabilitation is seeking qualified Physical Therapists to join our team providing high-quality care for patients in SNF settings. You will evaluate and treat patients according to an individualized plan of care, ensuring excellent service to patients and families.

You will collaborate with facility staff, supervise Physical Therapist Assistants, and complete timely, accurate documentation including evaluations, re-evaluations, discharges and daily progress notes.

Qualifications

• Evaluating new patients, developing plans of care and setting measurable, functional goals.
• Providing excellent care for our patients.
• Collaborating with facility staff to ensure excellent care for your patients.
• Supervising Physical Therapist Assistants.
• Completing timely and accurate documentation including evaluations, re-evaluations, discharges and daily progress notes.

• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
