Job Title: PRN Physical Therapist

Responsibilities and Duties:

• Develop and revise plans of care in consultation with physicians and care team members.
• Provide therapeutic treatments and evaluate equipment needs as applicable.
• Participate in clinical conferences and inservice programs.
• Conduct onsite supervisory visits of physical therapy assistants.
• Advise and consult with families and other agency personnel as applicable.
• Prepare clinical and progress notes.
• Protect and respect patient and coworker confidentiality and privacy rights.
• Evaluate outcomes of care.
• Plan for patient discharge.

Qualifications and Skills:

• Current Texas PT License.
• Current CPR certification.
• Valid Texas Driver's License and automobile insurance.
• Knowledge of WellSky EMR software is a plus.

Compensation: Pay per visit, including mileage reimbursement and additional pay points for drive time.
