Job summary

Enhance Rehabilitation is seeking qualified Physical Therapists to join our dynamic team in Joliet, IL. As a Physical Therapist, you will evaluate and treat patients in a skilled nursing facility following an individualized plan of care.

You will also collaborate with facility staff to ensure the highest quality patient care and excellent service for patients and their families. You will be responsible for evaluating new patients, developing care plans with measurable goals, educating patients

Responsibilities

• Evaluate and treat patients in a SNF setting according to an individualized plan of care.
• Provide excellent care for patients and customer service to patients and family members.
• Collaborate with facility staff to ensure high quality patient care.
• Evaluate new patients, develop plans of care and set measurable functional goals.
• Treat patients, address concerns and educate them on injury, progress and outcomes.
• Supervise Physical Therapist Assistants and supervise daily progress.
• Re-evaluate patient progress, revise plans of care and reset goals.
• Complete timely and accurate documentation including evaluations, re-evaluations, discharges and daily progress notes.
