Job summary

Enhance Rehabilitation is seeking qualified Physical Therapists to join our growing team in SNF settings. You will evaluate and treat patients following an individualized plan of care, aiming to improve function and independence.

Responsibilities include developing plans of care, setting measurable goals, supervising Physical Therapist Assistants, and documenting evaluations, progress notes, and discharges with accuracy and timeliness.

• Complete timely documentation: evaluations, re-evaluations, discharges and daily progress notes.

• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
