Job summary

Enhance Rehabilitation in Chicago is seeking qualified Physical Therapists to join our team in SNF settings. You will evaluate and treat patients following an individualized plan of care, deliver high‑quality care, and collaborate with facility staff to ensure excellent patient outcomes.

You will also supervise Physical Therapist Assistants, perform re‑evaluations, adjust plans of care, and complete timely documentation including evaluations, re-evaluations, discharges and daily progress notes.

• Complete timely and accurate documentation of evaluations, re-evaluations, discharges and progress notes.

• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
