Job summary

Oasis Therapy Services in Green Bay, Wisconsin, is seeking a Physical Therapist (PT) to provide one-on-one rehabilitation in an assisted living and memory care setting. This PRN role uses the Net Health EMR to document care and coordinate with a supportive care team.

The ideal candidate holds an active Wisconsin PT license, is trained in CPR, and has strong communication and organizational skills. Experience in geriatric or rehab settings is preferred.

Qualifications

• Active and current Wisconsin Physical Therapist license.
• Graduate of an accredited Physical Therapy program.
• Current CPR certification.
• Strong communication, organization, and documentation skills.
• Geriatric, assisted living, skilled nursing, or rehabilitation experience is preferred.

Responsibilities

• Evaluate mobility, strength, balance, gait, and fall risk.
• Develop and provide individualized treatment plans to improve safe mobility and independence.
• Monitor progress and adjust treatment plans as needed.
• Educate residents and caregivers on exercises, transfers, fall prevention, and assistive devices.
• Complete accurate Net Health documentation and collaborate with the care team.
