Job summary

Oasis Therapy Services is seeking a Physical Therapist (PT) to provide one-on-one rehabilitation services in an assisted living and memory care setting in Beaver Dam, Wisconsin.

Responsibilities include evaluating mobility, strength, balance, gait and fall risk; developing individualized treatment plans; monitoring progress; educating residents and caregivers; and completing Net Health documentation with the care team.

Qualifications

• Active and current Wisconsin Physical Therapist license.
• Graduate of an accredited Physical Therapy program.
• Current CPR certification.
• Strong communication, organization, and documentation skills.
• Geriatric, assisted living, skilled nursing, or rehabilitation experience is preferred.

Responsibilities

• Monitor progress and adjust treatment plans as needed.
