Job summary

Enhance Rehabilitation is seeking qualified Physical Therapists to join our team in SNF settings. You will evaluate and treat patients following an individualized plan of care, collaborate with facility staff, and provide education to patients and families.

You will also supervise Physical Therapist Assistants, update plans of care as progress is observed, and ensure timely documentation of evaluations, re-evaluations, discharges, and daily progress notes.

• Completing timely and accurate documentation including evaluations, re-evaluations, discharges and daily progress notes.
