Job summary

Enhance Rehabilitation is seeking qualified Physical Therapists to join our team in a SNF setting. You will evaluate and treat patients following an individualized plan of care, collaborating with facility staff to provide high-quality care and superior patient outcomes.

You will also supervise Physical Therapist Assistants, re-evaluate progress, adjust plans of care, and document evaluations, re-evaluations, discharges, and daily notes to ensure accurate records and continuity of care.

• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
