Qualifications

• Bachelor’s Degree in Pharmacy is required; Pharm D preferred.
• One year of long-term care experience is desirable.
• Current and valid pharmacist license in the state of practice.

Responsibilities

• Evaluate physician medication orders for appropriateness, dosage, interactions, and administration routes.
• Provide clinical recommendations to physicians and healthcare staff on therapy changes.
• Maintain up-to-date patient/resident profiles with medical history, labs, and medications.
