Job summary

PharMerica in Honolulu, HI is seeking a part-time Pharmacist to join a closed-door, long-term care pharmacy team. The role involves reviewing orders for safety, advising clinicians, maintaining resident profiles, and ensuring regulatory compliance.

Qualifications

• Bachelor’s degree in Pharmacy; PharmD preferred.
• Active pharmacist license in the state of practice.
• 1 year experience in long-term care preferred.
• Knowledge of CMS, DEA, FDA guidelines and regulatory standards.

Responsibilities

• Evaluate physician medication orders for drug appropriateness, correct dosage, potential drug interactions, and proper route of administration.
• Provide clinical recommendations to physicians and healthcare staff regarding medication therapy changes and optimizations.
• Maintain up-to-date patient/resident profiles including medical history, lab results, and current medications.
• Ensure laboratory tests required for monitoring high-risk medications are ordered, reviewed, and communicated to the team.
• Collaborate with physicians, nurses, and other healthcare professionals to provide clinical and technical support.
• Document all communications and clinical interventions in the patient’s medical record.
• Implement and enforce policies for safe handling, dispensing, and oversight of medications.
• Ensure compliance with state and federal regulatory requirements related to pharmacy practice.
• Report medication-related errors and adverse drug events to the Pharmacy Manager.
