Job summary

PharMerica’s closed-door, non-retail pharmacy in Lihue, HI seeks a PRN Pharmacist to join a dedicated team serving long-term care and senior living clients. You’ll evaluate orders, provide clinical recommendations, and maintain resident profiles while ensuring safety and regulatory compliance.

Qualifications

• Bachelor’s Degree in Pharmacy or PharmD preferred.
• Active pharmacist license in the State of practice.
• Experience in long-term care is desirable.
• Knowledge of drug therapy guidelines, safety practices, and regulatory standards.

Responsibilities

• Evaluate physician medication orders for appropriateness, dosing, interactions, and route.
• Provide clinical recommendations on therapy changes and optimizations.
• Maintain up-to-date patient/resident profiles including medical history, labs, and medications.
• Ensure monitoring labs for high-risk medications are ordered, reviewed, and communicated.
• Collaborate with physicians, nurses, and other professionals to provide clinical support.
• Document communications and interventions in the patient chart.
• Implement and enforce safe handling, dispensing, and overall medication management policies.
• Comply with state and federal regulations related to pharmacy practice and medication management.
• Report medication-related errors to Pharmacy Manager.
• Educate clinical team and agency staff on safe medication procedures.
• Perform other pharmacy-related tasks as assigned.
