Qualifications

  • Pharmacist must hold a Doctorate Degree in Pharmacy.
  • Basic Life Support (BLS) Certification required.
  • Knowledge of state and federal pharmacy laws and CMS guidelines.

Responsibilities

  • Maintains knowledge of changes to state and federal law affecting Pharmacy.
  • Maintains CMS rules and regulations for Critical Access Hospitals and Long-Term Care.
  • Assists in monitoring psychotropic medication use in nursing homes and implements action plans.
  • Performs chart reviews in the first 7 days and monthly thereafter for long-term care and swing bed patients.
  • Assists with bedside medication verification and adverse drug reaction reporting.
  • Assists with medication reconciliation and maintaining pharmacist competency and continuing education.
  • Assists with the 340b program and with HIM billing as needed.
  • Manages narcotic counts and perpetual inventory of controlled substances.
  • Serves as a resource for medical and nursing staff and ensures adequate inventory.
  • Participates in annual pharmacy inventory at fiscal year end.

Job Summary

WCHS is seeking PRN Pharmacist in our Hospital Pharmacy, to provide medication therapy management services in collaboration with other providers to assure that medication therapy is safe, effective, efficient, and cost-effective. Pharmacy service provided includes interdisciplinary patient care, medication reconciliation, medication therapy monitoring, long-term care consultation, medication orders management, patient education, drug information, drug dosing, and education activities. The Pharmacist performs both clinical and distributive tasks necessary to ensure care of patients. The pharmacist acts independently within the scope of their license and area of expertise with the help of policies, procedures, and practice guidelines with support from the Pharmacy Manager.
