Qualifications

• Must be a licensed Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) in the practicing state with training or experience in caring for chronically or terminally ill patients.
• Maintains active DEA license.
• Palliative care fellowship required.
• Familiarity with hospice regulations and Medicare requirements.

Responsibilities

• Develop and maintain Plan of Care for each patient, including discharge planning discussions with care team, patients, and families.
• Participate in prognosis evaluation and certification/recertification.
• Sign death certificates as needed.
• Conduct daily patient rounds and set consistent rounding schedules with interdisciplinary team.
• Ensure proper documentation of eligibility for inpatient care.
• Provide education and training to interdisciplinary team and new staff.
