Job summary

AccentCare is seeking a palliative care visit physician to provide comprehensive medical oversight and direct patient care within the Memorial Healthcare system. This role requires conducting patient rounds, coordinating physician coverage, and contributing to discharge planning while offering clinical leadership and education to the care team.

Qualified MDs/DOs must have a palliative care fellowship, active DEA license, and familiarity with hospice regulations and Medicare requirements.

Qualifications

  • MD/DO licensed in the practicing state with palliative training or experience.
  • Active DEA license.
  • Palliative care fellowship completed.
  • Experience with hospice regulations and Medicare requirements.
  • Strong clinical judgment and communication skills.

Responsibilities

  • Develop and maintain Plan of Care for each patient, including discharge planning discussions with care team, patients, and families.
  • Participate in prognosis evaluation and certification/recertification.
  • Sign death certificates as needed.
  • Conduct daily patient rounds and set consistent rounding schedules with interdisciplinary team.
  • Ensure proper documentation of eligibility for inpatient care.
  • Provide education and training to interdisciplinary team and new staff.
