Job summary

Qualifications

• Must be licensed MD or DO in the practicing state with training in chronically or terminally ill care.
• Active DEA license required.
• Palliative care fellowship required.
• Knowledge of hospice, Medicare requirements, and end-of-life care.

Responsibilities

• Develop and maintain Plan of Care for each patient, including discharge planning discussions with care team, patients, and families.
• Participate in prognosis evaluation and certification/recertification.
• Sign death certificates as needed.
• Conduct daily patient rounds and set consistent rounding schedules with interdisciplinary team.
• Ensure proper documentation of eligibility for inpatient care.
• Provide education and training to interdisciplinary team and new staff.
