Job summary

Care Guide Partners PACE in Elizabethtown, KY is seeking a PRN Nurse Practitioner to assess, initiate, provide, and coordinate direct care for participants in the CGPP program. Most visits occur in the clinic or day center, with occasional home, long-term care, assisted living, or hospital settings.

You will collaborate with the interdisciplinary team, support education and outreach, and may take on additional clinical or administrative duties at the Medical Director's direction.

Qualifications

• Licensure (or eligibility) for advanced nursing practice in KY, including DEA and controlled substance permits.
• Minimum of one year experience in advanced nursing practice.
• Knowledge of physical, mental, and social needs of frail older adults.
• Minimum 1 year of experience with frail/elderly population or training upon hire.
• Basic Life Support certification.
• Flexibility to travel within service area and provide infrequent home visits.
• Proficiency with electronic medical records.
• Ability to work independently and as part of a team; high autonomy for clinical decisions.

Responsibilities

• Assess, initiate, provide and coordinate direct care for CGPP program participants.
• Seen primarily in clinic or day center; may visit homes, long-term care facilities, assisted living facilities or hospitals.
• Coordinate care with the interdisciplinary team and other medical providers.
• Participate in education, outreach and leadership activities for the communities served.
• May be assigned additional patient care or administrative duties at the discretion of the Medical Director and/or Executive Director.
