Benedictine is seeking an Occupational Therapist to evaluate, plan, and implement therapy interventions for residents. The role emphasizes developing and revising care plans, setting functional goals, and coordinating discharge planning to meet both short- and long-term resident needs.
The successful candidate will educate the health care team, residents, families, and caregivers on the care plan to ensure safe and effective therapy outcomes.
The Occupational Therapist is responsible and accountable for residents' evaluation, ongoing assessment, planning, and implementation of therapy interventions.
Develops and revises plan of care based on the residents' needs and establishes functional goals, outcomes, and discharge plan. Plans for residents' discharge and anticipates for long term and/or short term residents' needs. Educates other health care team members, residents, family, and caregivers on information regarding the care plan for the residents.
