Job summary

The Occupational Therapist at Benedictine is responsible for evaluating residents, ongoing assessment, planning and implementing therapy interventions, and developing functional goals and discharge plans. They plan for discharge and anticipate for long term and short term needs, while educating the care team and families on the care plan to ensure continuity of care.

This casual/PRN role in Wisconsin requires current state OT licensure in good standing and a bachelor’s degree, with benefits

Qualifications

• Current state licensure as an Occupational Therapist, in good standing.
• Bachelor’s degree.

Responsibilities

• Evaluate residents and perform ongoing assessment and planning of therapy interventions.
• Develop and revise care plans with functional goals, outcomes, and discharge plans.
• Educate residents, families, and caregivers on the care plan and coordinate with the health care team.

Develops and revises plan of care based on the residents’ needs and establishes functional goals, outcomes, and discharge plan. Plans for residents’ discharge and anticipates for long term and/or short term residents’ needs. Educates other health care team members, residents, family, and caregivers on information regarding the care plan for the residents.
