Job summary

St. Gabriel's Community in Minnesota is seeking an Occupational Therapist to evaluate residents, monitor progress, and implement therapeutic interventions. You will develop and revise plans of care, set functional goals, and plan for discharge while anticipating long- and short-term needs.

The role includes educating the health care team, residents, families, and caregivers about the care plan to ensure coordinated, patient-centered care across the continuum.

Qualifications

• Current state licensure as an Occupational Therapist in good standing.
• Bachelor’s degree required.
