St. Gabriel's Community seeks a Physical Therapist to evaluate residents, perform ongoing assessments, and implement therapy interventions.

The role includes planning and revising care plans, coordinating discharge needs, and educating the care team, residents, families, and caregivers about the plan of care.

Qualifications

• Bachelor's degree required.
• Current state licensure in Physical Therapy in good standing.

Overview

The Physical Therapist is responsible and accountable for residents’ evaluations, ongoing assessments, and the planning and implementation of therapy interventions.

Responsibilities

Develops and revises plan of care based on the residents’ needs and establishes functional goals, outcomes, and discharge plans. Plans for residents’ discharge and anticipates for long term and/or short term residents’ needs. Educates other health care team members, residents, family, and caregivers on information regarding the care plan for the residents.
