Shadelands Post Acute in Walnut Creek, CA is seeking a PRN Occupational Therapist (OT) to join our in-house therapy team. The role offers flexible PRN hours, weekend availability, and the opportunity to help residents regain independence through ADL-focused therapy.

Qualificações

• Current and active California Occupational Therapist (OT) license.
• Strong clinical assessment and treatment-planning skills.
• Excellent communication and interpersonal skills.
• Ability to work effectively within an interdisciplinary team.
• Commitment to compassionate, resident-centered care.
• Weekend availability preferred.

Responsabilidades

• Evaluate residents and develop individualized occupational therapy treatment plans.
• Provide skilled OT interventions focused on ADLs, functional mobility, safety, and independence.
• Monitor resident progress and modify treatment plans as appropriate.
• Supervise and provide clinical guidance to COTAs.
• Collaborate with nursing, physicians, therapy professionals, and other interdisciplinary team members.
• Participate in care planning and discharge planning.
• Educate residents, families, and caregivers on treatment strategies and techniques.
• Maintain accurate, timely, and compliant clinical documentation.
