Job summary

Edenbrook of Sheboygan, part of Eden Senior Care Network, is currently seeking a PRN Occupational Therapist to provide rehabilitation services at one or more sites in Wisconsin. Join a growing healthcare management company founded to support skilled nursing and assisted living communities.

Responsibilities include evaluating residents, supervising COTAs, and applying ADL/IADL interventions while guiding families and caregivers.

Qualifications

• Master's or Doctorate degree in Occupational Therapy (OT) from an accredited program.
• Active Wisconsin Occupational Therapist (OT) license in good standing.
• Experience in skilled nursing, home health, or rehabilitation settings preferred.

• Provide quality Occupational therapy services and initiate referrals as appropriate.
• Evaluate residents to gather data for treatment planning and conduct specialized evaluations.
• Supervise Certified Occupational Therapy Assistants (COTAs) in treatment plan implementation.
• Apply interventions to maximize safety and performance in ADLs and IADLs.
• Recommend adaptive equipment and train in its use.
• Educate family members and caregivers.
• Observe, note, and report progress toward long- and short-term goals.

Position: PRN Occupational Therapist

Edenbrook of Sheboygan, a skilled nursing facility in the Eden Senior Care Network, is now hiring an Occupational Therapist to provide rehabilitation services in one or more of our service locations!

Primary Job Responsibilities:

• To provide quality Occupational therapy services and initiates referrals when appropriate, in accordance with established standards of practice, therapy department procedures, and productivity standards.
• To evaluate residents to obtain data necessary for treatment planning and implementation and conduct specialized evaluations as indicated.
• To supervise and direct Certified Occupational Therapy Assistants (COTAs) in the implementation of the treatment plan.
• Apply interventions to maximize safety and performance in activities of daily living (ADL) and instrumental activities of daily living (IADL)
• Recommend adaptive equipment and train in its use
• Guide and educate family members and caregivers
• Observe, note, and report on progress of long and short term treatment goals
