Job summary

Edenbrook of Sheboygan, part of Eden Senior Care, LLC, is hiring a PRN Occupational Therapist to provide rehabilitation services at a skilled nursing facility.

You will evaluate residents, develop treatment plans, supervise COTAs, and implement interventions to maximize ADL/IADL independence.

Work with families and the care team to monitor progress and adjust care as needed; Wisconsin license required. Sign-on bonus is offered and scheduling is flexible to fit community needs.

Qualifications

• Must hold a Master’s or Doctorate in Occupational Therapy from an accredited program.
• Active Wisconsin OT license in good standing.
• Experience in skilled nursing, home health, or rehabilitation settings preferred.

Responsibilities

• Provide quality OT services and refer when appropriate per standards and department procedures.
• Evaluate residents to gather data for treatment planning and conduct specialized evaluations.
• Supervise COTAs in implementing the treatment plan.
• Apply interventions to maximize ADL and IADL performance.
• Recommend adaptive equipment and train staff in its use.
• Guide and educate family members and caregivers.
• Observe and report progress toward short- and long-term goals.
