Job summary

Heritage Court Post Acute Care in Scottsdale, AZ is seeking a PRN Occupational Therapist for our in-house rehab program. Join a stable and fun therapy team focused on geriatric recovery and quality care.

Pay is $55 per hour and schedule is flexible to support work-life balance. Responsibilities include screening and evaluating patients, developing plans of care, using standardized assessments, and delivering rehabilitative treatments.

Qualifications

• Licensed Occupational Therapist licensed to practice.

Responsibilities

• Screen/evaluate patients with cognitive and/or ADL impairments and develop appropriate plan of care.
• Use standardized assessments and evidence-based practice to support interventions.
• Provide rehabilitative, skilled and medically necessary treatment per regulatory requirements.
• Administer procedures as part of the rehabilitation plan (ADL retraining, exercises, modalities, assistive devices).
• Assess progress and adjust treatments in the clinical record.
• Evaluate treatment outcomes and refer as needed based on outcomes.
• Provide counseling to patients and families related to physical disorders.
• Communicate with supervisor and health team about progress and plans.
• Provide discharge summaries per regulatory requirements.
