Job summary

Vantage Health in Wakefield, MA is seeking a PRN Occupational Therapist to support skilled nursing rehabilitation. You will evaluate self-care, cognition, and upper-extremity abilities, and develop personalized plans to help residents regain independence.

Collaborative care with families and staff is essential. Responsibilities include documenting care, supervising COTA services, and adapting goals as residents progress.

Qualifications

• Active Massachusetts Occupational Therapist license.
• Strong assessment skills and sound clinical judgment.
• Clear communication and timely, accurate documentation.
• Skilled nursing rehabilitation experience is a plus; new graduates are welcome to apply.

Responsibilities

• Evaluate residents' self-care performance, cognition, upper-extremity abilities, and functional activity tolerance.
• Build individualized treatment plans that connect clinical needs with meaningful daily living goals.
• Provide hands‑on therapy for personal care tasks, functional transfers, and other activities within the OT plan of care.
• Trial adaptive equipment and teach strategies that promote safe participation and greater independence.
• Supervise COTA services, assess treatment outcomes, and revise goals as residents' needs change.
• Document care and collaborate with residents, families, nursing, and rehabilitation staff on discharge recommendations.
