Job summary

Evolve Therapy Services, LLC is seeking a compassionate Occupational Therapist to join an in-house Skilled Nursing Facility partner in Church Hill, TN. This PRN role focuses on planning and directing OT services to ensure high-quality resident care and compliance with standards.

Responsibilities include evaluating residents, creating treatment plans with goals, coordinating with the interdisciplinary team, and supervising COTA while maintaining accurate documentation and billing.

Qualifications

• OT degree earned and actively licensed in practicing state.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Plan, organize, develop and direct Occupational Therapy services in facility.
• Screen residents to determine best intervention or treatment.
• Evaluate residents and develop treatment goals and plans.
• Communicate findings and recommendations to residents, families and team.
• Establish resident therapy schedules with the team.
• Deliver therapy per treatment plan and document/ bill services.
• Oversee COTA and ensure documentation and treatments are compliant.
