Job summary

Stern Therapy Consultants is hiring a PRN Occupational Therapist at Autumn Lake at Union in Union, NJ. In this skilled nursing rehabilitation role, you will identify barriers to residents’ everyday activities and provide treatment and strategies to help them participate more independently.

You will assess self-care performance, develop individualized plans of care, use purposeful activities to improve daily living skills, and educate residents and caregivers on safety.

Qualifications

• Active New Jersey Occupational Therapist license.
• Strong evaluation, clinical reasoning, and treatment planning skills.
• Effective communication and accurate, timely documentation.
• Skilled nursing or post-acute experience is a plus; new graduates are welcome to apply.

Responsibilities

• Assess self-care performance, functional transfers, upper-extremity abilities, cognition, and activity tolerance.
• Develop individualized plans of care based on evaluation findings, resident priorities, and anticipated discharge needs.
• Use purposeful activities to improve daily living skills and address limitations affecting task completion.
• Recommend adaptive equipment and educate residents and caregivers on safe techniques and compensatory strategies.
• Monitor functional gains, revise treatment goals, and supervise COTA services.
• Maintain thorough clinical records and communicate recommendations to nursing and the interdisciplinary care team.
