Job summary

Stern Therapy Consultants seeks a PRN Occupational Therapist to provide individualized evaluations and treatment at Autumn Lake at Ocean View in Ocean View, NJ. The role focuses on skilled nursing rehabilitation and daily living tasks, with emphasis on functional goals and discharge planning.

Responsibilities include evaluating residents, delivering therapy for ADLs and transfers, recommending adaptive equipment, and reviewing progress with the care team.

Qualifications

• Active New Jersey Occupational Therapist license.
• Strong clinical reasoning and individualized treatment planning.
• Clear communication and timely documentation.

Responsibilities

• Examine how physical abilities, cognition, and activity tolerance affect daily living performance.
• Establish functional goals and plans of care reflecting resident preferences and discharge needs.
• Deliver therapy for ADLs, upper-extremity function, and transfers.
• Recommend adaptive equipment and train residents and caregivers in safe techniques.
• Review progress, modify treatment plans, and supervise COTAs.
• Complete thorough documentation and coordinate care with nursing and rehab teams.
