Job summary

Stern Therapy Consultants is seeking a PRN Occupational Therapist for Autumn Lake at Scotch Plains to help skilled nursing residents improve self-care, adapt to functional changes, and prepare for next care steps.

You will evaluate daily living skills, develop measurable goals, and work with COTAs and nursing staff to implement therapy across ADLs, upper-extremity use, and energy conservation.

Qualifications

• Active New Jersey Occupational Therapist license is required.
• Strong treatment planning and clinical reasoning.
• Effective communication and timely, accurate documentation.

Responsibilities

• Evaluate residents' ability to complete daily activities considering physical, cognitive, safety, and routines.
• Establish individualized treatment plans with measurable goals.
• Deliver therapy addressing dressing, bathing, toileting, transfers, and upper-extremity use.
• Identify adaptive equipment needs and teach independence strategies.
• Review treatment responses, update care plans, and provide clinical direction to COTAs.
• Document evaluations and services, educate caregivers, and contribute to discharge planning.

Stern Therapy Consultants is seeking a PRN Occupational Therapist at Autumn Lake at Scotch Plains to help skilled nursing residents improve self-care skills, adapt to functional changes, and prepare for their next level of care.

What You'll Do

• Evaluate residents' ability to complete everyday activities, considering physical function, cognition, safety awareness, and prior routines.
• Establish individualized treatment plans with practical, measurable goals.
• Deliver therapy addressing dressing, bathing, toileting, functional transfers, and upper-extremity use.
• Identify adaptive equipment needs and teach techniques that support independence and energy conservation.
• Review treatment responses, update care plans, and provide clinical direction to COTAs.
• Document evaluations and services, educate caregivers, and contribute to interdisciplinary discharge planning.
