Job summary

Creative Solutions In HealthCare is seeking a compassionate Per Diem Occupational Therapy Assistant (OTA) to join our in-house rehab team. You will provide person-centered OT services to residents, helping them improve independence and quality of life.

The ideal candidate holds an active Texas OTA license, a COTA credential, and prior SNF or long-term care experience. You will work under an OT, document thoroughly, communicate with families, and collaborate with care teams.

Qualifications

• Active Texas OTA license in good standing.
• COTA credential required.
• SNF or long-term care experience preferred.
• Current knowledge of occupational therapy treatment practices.
• Experience with Net Health Electronic Documentation System preferred.
• Strong communication and teamwork skills.

Responsibilities

• Plan and administer occupational therapy treatments to residents to relieve pain and improve function.
• Provide services under supervision of an Occupational Therapist as per physician orders.
• Relay information and referrals to the Occupational Therapist.
• Deliver treatments in accordance with the plan of care.
• Provide clinical support and instruction to patients, families, and caregivers.
• Monitor patient response and report changes as appropriate.
• Complete required documentation thoroughly and timely in line with policy and regulations.
• Attend required meetings designated by the Director of Rehab.
