San Joaquin Nursing Center is seeking a qualified Occupational Therapist to join our PRN team. The OT will evaluate and treat residents, collaborate with families and the interdisciplinary team, and document care in the medical record.

The role includes staff training and ongoing evaluations to support resident independence. Ideal candidates hold an OT license in California, with strong assessment, planning, and communication skills, and are comfortable working in a fast-paced skilled nursing

Qualificações

• Bachelor’s degree in Occupational Therapy is required; Master’s or Doctorate preferred.
• Current, active California OT license is required; licensure in good standing.
• New graduates welcome to apply; experience in skilled nursing a plus.
• Experience in skilled nursing or post-acute settings preferred.

Responsabilidades

• Evaluate residents within 24 hours of physician referral.
• Develop individualized treatment plans and obtain physician approval.
• Provide skilled occupational therapy services per physician orders.
• Supervise COTAs, therapy aides, and students as applicable.
• Communicate resident progress with the interdisciplinary team.
• Participate in Care Conferences and Rehabilitation Conferences.
• Document evaluations, daily treatment notes, and progress in records.
• Instruct families and staff on maintenance programs and caregiver techniques.
• Assist discharge planning and recommend appropriate DME.
• Report equipment issues promptly; maintain professional relationships.
