Qualifications

• Must have completed an OT educational program approved by the governing board.
• Must possess a current Occupational Therapy license.
• BLS certification required.
• Ability to become proficient with computer software applications used in patient care.
• Arizona Fingerprint Clearance Card required for Banner Home Care AZ employment.

Responsibilities

• Evaluates patient's functional abilities and creates a plan of care.
• Implements individualized OT treatment respecting patient needs.
• Documents patient progress according to standards and timelines.
• Communicates results to physicians, nurses, patients, and families.
• Assists in department operations and staff training as needed.
