Job summary

Home&Care is seeking a compassionate Occupational Therapist to join our home health team in Casper, Wyoming. You will deliver one-on-one rehabilitative care in patients’ homes, helping them regain independence and safely perform daily tasks.

Responsibilities include evaluations, individualized care plans, and collaboration with physicians and the interdisciplinary team to optimize outcomes. A rewarding role with autonomy and meaningful patient relationships.

Qualifications

• Degree from an accredited Occupational Therapy program.
• Current and unrestricted OT licensure.
• Current CPR certification.
• Good organizational and communication skills.
• Valid driver’s license, auto insurance, and reliable transportation.
• Home health experience is a plus.

Responsibilities

• Provide one-on-one rehabilitative care to patients in their homes.
• Evaluate patients’ daily living and functional abilities and develop care plans.
• Implement therapeutic interventions to improve independence and function.
• Collaborate with physicians and interdisciplinary team on Plans of Care.
• Document patient progress and maintain accurate records on the day of service.
• Participate in care coordination and discharge planning as needed.
• Train patients and families in home exercise and safety strategies.
