Job summary

Creative Solutions in Healthcare - West is seeking a compassionate Occupational Therapist (OT) to join our in-house rehab team on a PRN basis. You will assess residents, develop individualized treatment plans, and support independence and safety through meaningful activities and ADLs.

Ideal candidates hold an active Texas OT license, have SNF/long-term care experience, and can collaborate with an interdisciplinary team. Flexible scheduling and competitive compensation are offered.

Qualifications

• Active and valid Texas Occupational Therapy license.
• SNF/long-term care experience preferred.
• Knowledge of occupational therapy treatment practices.
• Experience with Net Health Electronic Documentation System.

Responsibilities

• Consult with patients to evaluate their condition and impact on daily activities.
• Develop individualized treatment plans to maximize ADLs/IADLs.
• Assess environments and recommend adaptations for safety and independence.
• Provide training on adaptive equipment and safety.
• Collaborate with the interdisciplinary team to adjust care plans.
• Document thoroughly and timely per policies and regulations.
