Job summary

Life Care Center of Evergreen is seeking a PRN Occupational Therapist to join a dedicated rehab team in Colorado. The OT provides direct rehab care to patients per physician referrals, adhering to Life Care standards and applicable regulations.

Qualifications

• NBCOT certification required upon hire or renewal as applicable.
• Graduate of an accredited OT program (BSOT/MSOT).
• Active state licensure in good standing throughout employment.
• CPR certification required upon hire or during orientation.

Responsibilities

• Follow established occupational therapy treatment plans for patients.
• Establish and modify measurable goals for patient progress.
• Oversee and evaluate care provided by OTAs and aides.
• Document patient charts in a timely, accurate manner.
• Utilize therapy software and communicate effectively with the team.
