Job summary

Nurturing Home Healthcare is seeking a compassionate Occupational Therapist to join our Home Health team on a PRN basis in Houston, TX. The OT will evaluate, develop, and implement individualized treatment plans to help patients perform daily living activities at home.

The role requires independent work, strong clinical skills, and collaboration with physicians, nurses, and caregivers to achieve optimal patient outcomes. Travel to patients' homes is required as part of a flexible schedule.

Qualifications

• Current Texas OT license.
• Graduate of an accredited OT program.
• Minimum of one year OT experience.
• Strong knowledge of OT principles, assessments, and techniques.
• Excellent clinical judgment and problem-solving skills.
• Strong verbal and written communication abilities.
• Ability to work independently and manage a flexible caseload.
• Current CPR certification or ability to obtain upon hire.
• Ability to maintain accurate and timely patient documentation.

Responsibilities

• Perform comprehensive occupational therapy evaluations in patients' homes.
• Develop individualized treatment plans based on physician orders and patient needs.
• Assess patients' ADLs and IADLs.
• Provide therapeutic interventions to improve strength, mobility, cognition, and independence.
• Educate patients and caregivers on adaptive techniques and home exercise programs.
• Recommend adaptive equipment and home modifications to enhance safety.
• Monitor progress and adjust treatment plans as needed.
• Collaborate with physicians, nurses, and other therapists to coordinate care.
• Identify changes in patient condition and communicate findings to the team.
• Complete accurate clinical documentation per agency policies.
• Participate in interdisciplinary team meetings and case conferences.
• Ensure compliance with Medicare regulations and company policies.
• Promote patient safety, independence, and improved quality of life.
