Continuum Home Health LLC seeks an Occupational Therapist to join its home health team in the Houston area. You will assess, plan, and implement therapies in patients' homes to promote independence across pediatrics to geriatrics.

The role involves creating individualized plans, training caregivers, and coordinating with nurses and therapists in a flexible, community-based setting. Part-time/PRN/Contract opportunities available.

Formación

• Strong knowledge of physiology, anatomy, and medical terminology relevant to occupational therapy practice.
• Experience across settings such as inpatient units, ICU, outpatient clinics, SNFs, and home health.
• Proficiency in discharge planning and case management to support outcomes.
• Ability to assist with ADLs and functional activities for patients of all ages.
• Familiarity with early intervention programs and school-based OT services.
• Experience with developmental disabilities.
• Skilled in developing tailored care plans for post-acute care.
• Ability to work in multidisciplinary teams including SLP, PT, nurses, and physicians.
• Excellent communication with patients and families.

Responsabilidades

• Conduct in-home OT evaluations to assess ADL performance, cognition, and motor skills.
• Develop individualized treatment plans based on goals, home environment, and conditions.
• Implement evidence-based interventions including ADL retraining and balance training.
• Train patients and caregivers in transfers, fall-prevention, and use of adaptive equipment.
• Provide home safety evaluations and modifications to support aging-in-place.
• Document visits and progress in Winnolas EMR.
• Collaborate with SN, PT, SLP, Social Work, and Aides for a unified plan of care.
• Participate in case conferences, recerts, and discharge planning.
