Choice Health At Home in Durant, OK is looking for a licensed Occupational Therapist (PRN) to assess and treat patients of all ages in a home health setting. You will develop and document a comprehensive Plan of Care, perform regular progress notes, and coordinate with the IDG to ensure high-quality care.

The role requires a valid OT license, CPR certification, and a valid driver’s license with auto insurance. Prior HomeCare HomeBase experience is preferred.

Formación

• Valid Occupational Therapy License.
• 1 year of occupational therapy experience preferred.
• CPR certification required.
• Valid Driver’s License & Auto Insurance.
• HomeCare HomeBase experience preferred.

Responsabilidades

• Perform clinical assessment and identify patient needs.
• Document and update Plan of Care, progress notes, and orders in a timely manner.
• Provide interventions to improve ROM, strength, coordination, and ADL performance.
• Participate in IDG care planning and coordination of care.
• Apply infection control and standard precautions during patient care.
• Assess need for adaptive equipment and environmental modifications.
• Communicate effectively with the care team to achieve outcomes.
• Meet continuing education requirements and promote quality of care.
