Job summary

Choice Health at Home in Okmulgee, OK is seeking a licensed Occupational Therapist to join as a PRN provider. You will evaluate patients across age groups in home health, develop and adjust care plans, and document patient progress to support quality outcomes.

The role requires an active OT license, CPR, and a valid driver's license with auto insurance. HomeCare HomeBase experience is preferred to coordinate care efficiently. This is an on-site opportunity serving the community.

Qualifications

• Valid Occupational Therapy License.
• 1 year of occupational therapy experience preferred.
• CPR certification.
• Valid Driver’s License & Auto Insurance.
• HomeCare HomeBase experience preferred.

Responsibilities

• Perform clinical assessment; identifies patient needs and appropriateness of patient.
• Documents, prepares, revises, and completes a Plan of Care, progress notes, phone orders and other clinical record documentation on an ongoing basis and in a timely manner.
• Accepts clinical assignments that are consistent with education and competence to care for patients/clients.
• Participates in the IDG care planning process.
• Apply concepts of infection control and standard precautions in coordination/performing patient care activities to prevent contamination and transmission of disease.
• Assess and facilitate through direct therapeutic intervention the patient's ROM, strength, coordination, need for and use of adaptive equipment and environmental modifications, functional mobility and safety issues as related to patient’s ADL performance needs. Fabricate and maintain adaptive equipment and hand splints.
• Uses effective interpersonal relations and communication skills; facilitates the use of these skills by other team members to achieve desirable outcomes.
• Evaluate patient occupational performance in the areas of self-care, work, leisure and rest. Assess sensory-motor neuromuscular, psycho-social and cognitive-perceptual components of occupational behavior. Assess physical, social and cultural components of the patient’s occupational environment.
• Meets mandatory continuing education requirements of the Agency and the licensing board.
• Promotes the Agency philosophy and administrative policies to ensure quality of care.
