Job summary

Choice Health at Home is seeking a licensed Physical Therapist to join our Durant, OK home health team. The PT will assess patients, develop and implement individualized treatment plans, and monitor progress to promote mobility and independence in the patient’s home.

The role requires valid PT licensure, accredited program completion, and CPR. Home health experience is preferred, and driving with a valid license and insurance is required to visit patients in their homes.

Qualifications

• Successful completion of an accredited physical therapy program.
• Valid Physical Therapy license (state licensure).
• CPR certification.
• Driver's license and auto insurance.

Responsibilities

• Perform and document an initial exam to identify problems and diagnosis before intervention.
• Plan, prepare, or carry out individually designed programs to maintain, improve, or restore physical functioning.
• Evaluate effects of treatment and adjust plans to maximize benefit.
• Maintain infection control/universal precautions in patient care.
• Ensure continuity of quality patient care with proper documentation.
• Provide effective communication to patient/family, team members and other health care professionals.
• Promote agency philosophy and administrative policies.
• Keep all documentation within company guidelines.
