Choice Health at Home in Clearwater, FL seeks a licensed Occupational Therapist for PRN coverage across ages. The therapist will assess, plan, implement and evaluate care, coordinate with the IDG, and ensure infection control and safe ADL support.

Requirements include a valid OT license, CPR, and a valid driver's license with auto insurance. One year of OT experience is preferred; HomeCare HomeBase experience is a plus. Excellent documentation and interpersonal skills are essential for success.

Formación

• Valid Occupational Therapy License.
• 1 year of occupational therapy experience preferred.
• CPR certification.
• Valid Driver’s License & Auto Insurance.
• HomeCare HomeBase experience preferred.

Responsabilidades

• Perform clinical assessment; identifies patient needs and appropriateness of patient.
• Documents, prepares, revises, and completes a Plan of Care and progress notes.
• Accepts clinical assignments consistent with education and competence.
• Participates in the IDG care planning process.
• Apply infection control concepts to patient care activities.
• Assess and facilitate ROM, strength, coordination and ADL performance; fabricate adaptive equipment.
• Utilize effective interpersonal relations to achieve outcomes.
• Evaluate occupational performance in self-care, work, leisure and rest.
