Job summary

Tennessee Quality Care is seeking a licensed Occupational Therapist (OT) for PRN Home Health roles serving Columbia, TN and surrounding counties. You will deliver one-on-one in-home therapy, helping patients regain independence and improve daily functioning.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• NBCOT certification and TN licensure required.
• Minimum two years of OT clinical experience preferred.
• Current CPR certification is required.
• Reliable transportation and valid driver's license.

Responsibilities

• Provide occupational therapy services per physician’s plan of care.
• Develop individualized treatment plans and functional goals.
• Communicate plans of care with physicians and team.
• Teach ADLs and retrain patients as needed.
• Fabricate splints and instruct adaptive equipment use.
• Train patients in prosthetic/orthotic device use as appropriate.
• Maintain accurate clinical documentation and patient updates.
• Notify changes in patient condition to the care team.
• Collaborate with other healthcare professionals for coordinated care.
