Job summary

Adoration Health is seeking a Home Health Occupational Therapist to join our team in Woodbury, TN. You will provide 1:1, patient-centered OT services in a home health setting, with opportunities to work independently while receiving team support.

The role supports coverage for Cannon & DeKalb Counties, with a PRN schedule and opportunities for advancement. Requirements include CAPTE-approved OT education, current TN OT license, and CPR certification; two years of adult home health experience are

Qualifications

• Graduate of a CAPTE-approved Occupational Therapy Education program.
• Current OT licensure in the state and CPR certification; CPR may be offered upon hire.
• Two years of adult occupational therapy experience; home health experience preferred.
• Excellent observation, verbal and written communication skills; problem solving; basic math skills.

Responsibilities

• Perform OT evaluation and treatment based on physician orders; recommend and procure adaptive equipment.
• Establish measurable goals and develop treatment plans toward independence.
• Maintain communication among treatment team, physician, patient and family.
• Maintain timely documentation for proper billing.
• Supervise OT assistants in patient care and related activities as per state act.
